How to schedule an appointmet in Nice with our ocular oncology team as a patient
To make an appointment at your doctor’s request or to seek a second opinion, please follow the steps below.
- Current last name, birth name, first name, date of birth
- Place of birth (city/town)
- Your phone number and residential address
- Name of your referring doctor, if applicable

- If requested by your doctor: You may have been given a medical prescription for your visit to Nice.
For long-distance travel (over 150 km one way), you should also have received a prior approval request form to send to the medical advisor of your local Health Insurance Fund (Caisse Primaire d’Assurance Maladie). If you receive no reply within 15 days after sending it, the request is considered approved. In case of refusal, you will receive a letter from Health Insurance. - If you are making the request yourself: Transportation costs will be at your own expense.

Important: You must follow the mode of transport prescribed unless you choose a less expensive option. For example, if your doctor has prescribed professional seated transport (taxi or light medical vehicle) but you are driven by a relative, you may request reimbursement for the cost of using your personal vehicle for medical reasons.
Please bring:
- Valid proof of ID (identity card, passport, or driver’s license)
- Your Carte Vitale (French health insurance card)
- Any medical documents or letters related to your condition
- A prescription for your current treatments (if applicable)
- Names of the physicians involved in your care (ophthalmologist, general practitioner)
Some documents can be uploaded in advance on Doctolib to facilitate their storage by the doctor who will see you.
A report of your consultation in Nice will be sent to your doctors at the end of your visit. You will also receive a copy of this report.
